REPORTS TO: Lead Care Coordinator, Director of Community Programs, CEO

Summary of Duties

The Outreach Care Coordinator is responsible for increasing community awareness and community connections to improve Palm Beach County residents’ access to Breast and Cervical Cancer Screening and Education. In addition to direct outreach, responsibility of the Outreach Care Coordinator is to assist the patient access health care, provide education, identify needed supports, and provide appropriate community resource linkage. Patients are screened to assess appropriate program enrollments. The care coordinator assists the patient navigate the health care system and provides support through care transitions. This includes referrals to Project Access volunteer provider network, community-based supports as well as County and Federal programs.

Minimum Qualifications

  • Graduation from an accredited university with a health-related background or related professional/educational experience
  • Community Health Worker Certification preferred
  • Min 2 years’ experience in related field
  • Fluency in English – Bilingual in Spanish or Creole preferred
  • Flexible in work hours including occasional evenings and Saturdays
  • Able to maintain confidentiality with all aspects of information in accordance with practice, State and Federal regulations
  • Self-disciplined, energetic, passionate, and innovative
  • Superb organization skills and attention to detail
  • Ability to prioritize multiple projects while meeting goals and deadlines
  • Ability to stay on task and meet deadlines with minimal supervision
  • Possess effective computer skills & communication skills with a focus on customer service

What You Will Do

  • Attend meetings both internally, virtually and throughout the community
  • Participate in applicable or requested continuing education opportunities
  • Participate in community outreach opportunities as directed
  • Assist patients through the healthcare system by acting as a patient advocate and navigator
  • Complete Assessments to determine appropriate program referrals
  • Facilitate application assistance for patients in need of access to healthcare and related supports
  • Coordinate continuity of patient care with external healthcare organizations and facilities, including referrals from the primary care provider to a specialty care provider
  • Prepare outcome reports as needed
  • Review medical records & manage requests
  • Promote clear communication amongst a care team and treating clinicians by ensuring awareness regarding patient care plans
  • Provide guidance and support to patients/clients; Set program expectations
  • Troubleshoot issues with care provided via program
  • Establish and maintain effective working relationships with staff and community partners
